Dental Insurance
Due to the increased enforcement of the privacy act, we are only able to obtain very limited or no information at all about your individual dental plans. This is particularly so in regards to routine work like check up exams, x-rays, and cleanings. Our treatment recommendations are guided by what’s best for your care, not by what your insurance allows.
As a result, we encourage you to contact your insurance company regarding your coverage. Please ask at the front desk for our “Ask Your Insurance Company” card. This card can be used to record your insurance information when contacting your insurance company and can be held on to for your future reference.

Consult Only Appointments
Payment is due the day of the consultation. For your convenience, we accept debit, Visa, and MasterCard.
Cancellations & No Shows
Please be courteous to us and other patients by informing us 2 business days in advance if you wish to reschedule or cancel your scheduled appointment with us. Cancellation/no shows will result in a fee.
Your First Visit
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist use by providing the following information at the time of your first appointment:

Your referral slip and any x-rays if applicable

A list of medications you are presently taking.

If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.
Radiograph (X-Rays)
If your referring dentist has taken radiograph (x-rays), they may have them forwarded to our office, yet we may need to take additional radiographs at your first appointment. Any appointments occurring after a 6 month time frame may be subject to another radiograph, and re-evaluation.
